You’re With Us! is a registered 501 (c)(3) non-profit organization and Department of Developmental Services (DDS) service provider which seeks to create better communities by harnessing the benefits of inclusion for individuals with disabilities. We believe that not only do people with disabilities need inclusion opportunities in order to thrive, but also that these opportunities allow them to contribute to their community in ways that have gone unrecognized in the past. Social connections are a basic human need. You’re With Us! helps with this by identifying, training and supporting college students, clubs, groups, and teams to be welcoming and supportive peers.
Position: Community Based Case Manager
Help us create better communities, improve inclusion on more college campuses, and impact many people’s lives! The Case Manager will be responsible for developing and supporting programming for clients to help them achieve goals that will contribute to creating the life they want with support through adult programming. Our program helps individuals develop independent living skills, engage in activities they are interested in and navigate the community in which they live to make these opportunities sustainable.
Responsibilities:
• Provide case management and skills training support to clients in areas such as career exploration, hard and soft employment skills, college campus inclusion, communication, decision-making, social skills, transportation goals, community safety and activities of daily living.
• Develop programming and curriculum on a case by case program that allows opportunities for clients to develop skills to achieve their goals in a safe and controlled environment
• Review case records and programming and curriculum on a regular basis to ensure compliance with You’re With Us! Standards.
• Provide transportation to participants in the candidate's own vehicle as needed.
• Participate in implementing and monitoring curriculum and treatment plans, completing all necessary programmatic documentation and reporting significant concerns to You’re With Us! in a timely manner.
• Develop and run group activities in the community.
• Act as an advocate for You’re With Us! clients and their families.
• Develop and maintain positive relationships with guardians, family members and other service providers.
• Perform other related work duties as assigned by senior You’re With Us! staff.
Required Qualifications:
• Associates Degree in Human Services or related field, or 3-4 years of supervisor experience within a day or residential program for individuals with disabilities.
• Direct Care experience working with individuals with Intellectual/Developmental Disabilities
• Excellent organizational, communication, and writing skills.
• Demonstrated ability to work both independently and collaboratively with a culturally diverse population of individuals, co-workers, care providers, family, community members and others.
• Ability to write clear, concise and accurate documentation.
• Ability to lift 30 pounds as needed.
• Valid driver’s license and acceptable driving record is required.
• Current First Aid, CPR and Crisis Intervention certificates or be able to obtain these within thirty (30) days of hire.
• Pass criminal background check(CORI) and National Fingerprint Background Check
Preferred Qualifications:
• Bachelor’s Degree in Human Services or related field preferred.
Compensation: $20.00 - $24.00 per hour
Our Mission And Vision
You’re With Us! creates inclusion opportunities for young adults with disabilities. The program identifies and trains college clubs, groups, and teams to be welcoming and supportive peers to the differently abled. The college group then creates a Positive Experience Design (PED) customized for the young adult to help one’s transition from school to adult life in the community. You’re With Us! believes that a meaningful life includes a home, a job, family, friends, and social opportunities with their peers - able and otherwise.
Our program is customized on an Individual-by-individual basis. Once we meet the individual and their family and learn more about their goals for their adult life and what they're hoping their life will look like, we will determine the support and skill development needed to reach those goals. All individuals who are receiving CBDS support from YWU are also connected to peer mentors through YWU to help facilitate opportunities for skill development and social inclusion in a college community. We match and support peer connections based on the individual’s interests and hobbies.
We locate possible opportunities through our college mentors to join college groups, teams, and clubs. YWU coordinates recruitment into the group; acts as the liaison between the family, school, and college/university; trains mentors of the college groups to welcome participants into their groups as they are, facilitates communication between all parties; facilitates group education and development of Positive Experience Design; gather data on impact and quality of life; create and coordinate social connections across campus for participants and families; facilitate possible job/career opportunities through college career resources; offer support services as needed through our licensed clinicians, and generally problem-solve to make these opportunities sustainable for the rest of our peer’s lives.
